Provider Referral

Your provider refers you and the DEEP™ coordinator will reach out to schedule your session. Just ask your care team to send a referral.

Diabetes Support Group

A welcoming monthly group where you can share, learn, and connect with others managing diabetes. The group meets once a month for 10 months, one hour each session.

Grocery Store Tour

Join a certified educator for a one-hour guided tour of the grocery store. Learn how to read labels, choose smart options, and shop for meals that support healthy blood sugar.
