In-Person Medical Record Requests and Retrieval

At this time to ensure the safety of our patients and staff due to COVID-19, we are restricting in-person access to request and retrieval of medical records.

Please use the online options available to request records via email, mail, fax, phone, via your provider or by viewing through the patient portal.

You may request that your records be sent directly to your provider via fax or the provider can call and request the records directly via phone, fax or email.

If you have questions or would like additional information, please call us at (760) 499-3668 and our staff will be happy to assist you.

If you have not received a response to your request within 10 business days, please call us immediately at (760) 499-3668.
