Preparing Patients for Discharge

Before you go home from the hospital, you will be given personal, customized instructions about medications, diet, and when to resume your daily activities. Even when you are ready to live the hospital, you may not be ready to resume your normal lifestyle yet. It’s important to be diligent about your recovery when you leave the hospital, and we make sure you have all the knowledge and resources you need to make this possible.

Our Case Management & Social Services Department coordinates additional discharge services with your physician, family members, and community agencies. For example, if you need medical equipment, rehabilitation, or long-term care, a Case Manager or Social Worker can coordinate these services or direct you to the appropriate agency for assistance. We can also provide referrals to any medical specialists outside of the hospital who can aid in your long-term recovery.
