Why It Matters

A yearly check-in keeps your child on track.

An annual physical is more than a school-form signature. It's a yearly check-up to make sure your child is growing, developing, and feeling well, and to catch anything that needs attention early.

During the visit, your provider will measure height and weight, check vital signs, review vaccines, and talk with you about diet, sleep, activity, school, and any concerns you have. For teens, your provider will also talk through changes during puberty and answer questions privately if needed.

Insurance covers one physical every 12 months from your last visit. The best time to book is when you're due, not when paperwork is suddenly required for school or sports.

You'll need to be an established patient, or willing to become one during the visit, to schedule a physical. New patient packets are required at the first appointment.

Schedule an appointment with the clinic you'd like to visit

Pick whichever clinic is most convenient for your family. New patients: we'll send a packet to fill out before your visit.

Fill out your forms

Fill out required forms online to save time at check-in. Bring any school or sports forms you need signed.

Come in for the visit

Plan on 45–60 minutes for a new patient visit, less for return visits. Bring your child's insurance card and completed forms.

Sports Physicals

Cleared to play.

Most schools and youth sports leagues require a sports physical before your child can practice or compete. It's a focused check-up that confirms it's safe for them to take the field, the court, or the mat.

During a sports physical, your provider reviews your child's health history, checks heart and lungs, looks at joints, muscles, and flexibility, and signs the league or school form when everything checks out.

Sports physical or annual physical? An annual physical covers everything a sports physical does and more. If your child is due for their yearly check-up anyway, schedule the annual visit. One visit covers both needs.

What to bring

Your child's insurance card, the school or league sports form, and a list of any current medications. Wear comfortable clothes that make it easy to move and stretch.
